8-A.(1) - Garbage and trash storage containers are unacceptable.
9-A.(8) - Lights are not shielded in the laundry areas and the linen storage facilities.
9-A.(7) - Toxic materials are not properly labeled and stored.
9-A.(6) - No Material Safety Data Sheet available on the premises.
2-A.(8) - Torn and/or soiled bedding, mattresses, sheets or pillowcases are being used.
2-C.(8) - A supply of toilet tissue was not provided at each toilet, easily cleanable receptacles are not provided for waste materials, receptacles in toilet rooms for women are not covered and/or waste receptacles not emptied at least once per day or more when needed to prevent an excessive accumulation of waste.
2-C.(9) - Signs requiring employees to wash their hands after using the toilet are not posted in all toilet rooms open to the public and employees.
2-C.(5) - An adequate supply of sanitary paper or cloth towels, or an approved hand drying device, is not available and conveniently located near the lavatory.
2-C.(3) - An adequate supply of hand soap or detergent is not available at each lavatory.
