4.G - The tattoo practitioner did not wash his or her hands with antibacterial soap and water and/or did not did not use a single use disposable towel to dry his or her hands before starting to tattoo.
4.P - Single service items not used for each patron, are not disposed of in accordance with state and local laws and/or the establishment is not registered with the Department of Environmental Protection under the Biomedical Waste Disposal Rules.
4.S - Tattoo practitioner did not provide proper written and verbal aftercare instructions to the client after a tattoo procedure.
6.C - Patron records not retained and made available for at least two (2) years and/or a receipt with required information was not issued to a client at time of tattoo procedure visit.
