4.S - Tattoo practitioner did not provide proper written and verbal aftercare instructions to the client after a tattoo procedure.
4.W - A medical history of the client not obtained, the client not advised of possible complications from being tattooed as required and/or the client not questioned about any history of latex allergy.
6.C - Patron records not retained and made available for at least two (2) years and/or a receipt with required information was not issued to a client at time of tattoo procedure visit.
