Establishment: 28096 - WADLINGTON, SEAN Inspection Date: 10/02/2020 Inspection Purpose: FOLLOW_UP_FULL Status: Failed: 6 Critical, 3 Non-critical violations
Violations:
1.A - Operating without a license.
4.B - Body piercing practiced on a person who is less than 18 years of age without parental or guardian consent and/or the body piercer did not obtain proper identification from the parent(s) or guardian(s) of a person less than 18 years of age.
5.B.4 - The sterilizer is not spore tested at least once each month, packaged sterilized instruments not monitored for sterilization by the use of chemical/heat sensitive indicators, packaged sterilized instruments not re-sterilized after 6 months before use, spore test records not maintained for one year and/or sterilizer spore test records not available for inspection.
5.A - Clean sterile needles and similar instruments are not properly stored in closed, rigid containers or a storage cabinet when not in use and/or the storage cabinet is not maintained in a sanitary manner.
4.P - Jewelry not single customer use only and/or not individually packaged and sterilized prior to use.
5.B.1 - Autoclave not used.
6.A - Records not obtained as required.
6.C - Records not being retained for a minimum of 2 years, receipts not issued to each patron setting forth the name and address of the establishment, contact number and the piercer's name and signature and/or records not made available to the Department when requested.
