5.B.4 - Autoclave not spore tested at least once each month, packages used for sterilizing instruments do not have chemical/heat sensitive indicators, sterilized instruments not re-sterilized after six months and/or spore test records not retained and available for inspection for a minimum of one year.
6.C - Patron records not retained and made available for at least two (2) years and/or a receipt with required information was not issued to a client at time of tattoo procedure visit.
