12-C - The potable water system is not installed to prevent the possibility of backflow and backsiphonage at all fixtures, equipment and faucets where hoses are attached.
11-A.(3) - Screen doors to the outer air are not self-closing; and screens for windows, doors, skylights, transoms, and other openings to the outer air are not tight-fitting and free of breaks.
9-A.(8) - Lights are not shielded in the laundry areas and the linen storage facilities.
2-A.(10) - Furniture, fixtures, carpets and other accessories are not maintained and/or are not clean.
2-A.(14) - Appliances are not properly cleaned and/or not properly maintained.
2-A.(7) - Sheets and pillowcases are not clean.
2-C.(8) - A supply of toilet tissue was not provided at each toilet, easily cleanable receptacles are not provided for waste materials, receptacles in toilet rooms for women are not covered and/or waste receptacles not emptied at least once per day or more when needed to prevent an excessive accumulation of waste.
