11-A.(3) - Screen doors to the outer air are not self-closing; and screens for windows, doors, skylights, transoms, and other openings to the outer air are not tight-fitting and free of breaks.
4-A.(1) - Establishment is not in compliance with NFPA 101 Life Safety Code.
2-B.(1) - The floors, walls, ceilings, windows and ventilation in common areas associated with sleeping rooms are not clean and/or not properly maintained.
2-A.(13) - Drinking glasses have not been washed and sanitized after each day of occupancy and/or clean glasses, not otherwise protected, are not inverted on a clean surface.
2-A.(12) - The lodging establishment failed to wash, rinse and sanitize the cooking and multi-use eating and drinking tableware prior to use by succeeding guests.
